The laws regarding vote-by-mail have been changed recently - the latest information can be found on the website of the Leon County Supervisor of Elections
here.
Any registered voter can request a Vote-by-Mail ballot and you don't need and excuse or reason. A single request is good through the end of the current election cycle, but will need to be renewed for each subsequent cycle. You may request a Vote-by-Mail ballot in any of the following ways:

Leon Votes and a Vote-by-Mail Ballot Request Form will appear in PDF format. You may print it, fill it out and mail it by U.S. mail to the
Leon County Supervisor of Elections office, P.O. Box 7357, Tallahassee, FL 32314-7357. You may do this for yourself, for another person or someone can do it for you.
2. You may call the Supervisor of Elections office at 850-606-8683 between 8:30 and 5:00 Monday – Friday to request a vote-by-mail ballot.
3. You may fax a request for a mail ballot to 850-606-8601. You
must include your name, date of birth and either the last 4 digits of your social security number or your driver's license number.
